Drug in trial works wonders for lung cancer patients

A new drug has shown major promise on its ability to suppress advanced lung cancer into a manageable chronic disease, according to researchers from the Chinese University of Hong Kong's Faculty of Medicine (CU Medicine). In a global clinical trial spanning seven years, CU Medicine enrolled 296 advanced lung cancer patients with the ALK -- Anaplastic Lymphoma Kinase -- genomic mutation. The ALK mutation is a genetic alteration of DNA that causes lung cells to grow abnormally, and ultimately become cancer cells. Around half, or 149, were treated with the drug lorlatinib, a third-generation targeted therapy. The others were administered a first-generation agent as control. Of those patients who took lorlatinib, 55 percent remained progression-free after seven years -- the longest-ever period achieved by lung cancer targeted therapies. By comparison, only 3 percent of the other group had reached that mark, with their median PFS, or progression free survival, standing at just over nine months. Researchers also found that 92 percent of patients who received lorlatinib were free of intracranial progression after seven years, despite a high chance of lung cancer metastasising to the brain. "If [the patient] is doing well and progression-free at the second year [of treatment], there is a close to 80 percent chance that [the patient] will continue to be well and progression-free at the end of seven years," said Professor Tony Mok, who chairs the Department of Clinical Oncology at CU Medicine. Lorlatinib has also been found to be safe for long-term use, barring common side effects including high cholesterol, edema, weight gain and cognitive dysfunction. Professor Mok noted that most of these side effects can be mitigated by medication or lifestyle changes. Cognitive dysfunction, meanwhile, can be effectively managed by reducing the dosage of lorlatinib without compromising its therapeutic efficacy, he added. Edited by Raymond Yeung

'Pay trend survey crucial in determining 2pc pay rise'

Civil service chief Ingrid Yeung on Wednesday said a pay trend survey that the government refers to in deciding a final pay rise was crucial in its decision-making process, even though the Executive Council's latest proposal fell short of the indicators for some staff. The blanket two percent pay rise will be backdated to April 1 if the Legislative Council gives the green light. The pay trend survey recently pointed to a 4.12 percent and 2.64 percent pay rise for senior and middle-ranking civil servants, respectively, and 1.17 percent for juniors. But Yeung told an RTHK radio show that the findings still played an important role. “The net indicators from the pay trend survey give us a very useful reference — specifically, the salary trends in the private sector, broken down by three different pay grades. What is the trend like? How is the trend moving? That is very useful,” she said. “I believe no one would want to lose this reference, and instead just pull a number out of thin air, that would be even less scientific and entirely groundless.” Yeung said the city’s 170,000 civil servants should be given a pay rise this year to recognise their efforts, maintain their morale and ensure that their salary is comparable with the private sector. Even though the SAR’s economic situation is good in the long-run and the government financial situation is improving, the minister said the city's development needs pose a big burden on the government's finances. “The [outcome of the] Middle East conflict has not become clear. The government, due to the situation there, initiated subsidies for some industries, otherwise the pressure would be passed on to the citizens. As for how things will be in the future, no one knows yet. Most importantly, it is not as though everything will be fine once it is over,” she said. “It actually reminded us that Hong Kong, as an externally oriented economy, is particularly vulnerable to such a blow. We could be severely impacted by international economic developments and financial fluctuations. So we have to be cautious in handing the government's finances.” Yeung went on to say that the relief measures were an unexpected step, which constitute an additional financial commitment. Blanket 2pc pay rise on table for civil servants

The Chief Executive-in-Council proposed a two percent across-the-board pay increase for the city’s 170,000 civil servants on Tuesday, backdated to April 1. If approved by the Legislative Council, the pay rise will cost about HK$6 billion extra. Secretary for the Civil Service Ingrid Yeung said the government had taken a prudent approach and considered all factors in a comprehensive manner in reaching this decision. “[We have taken into account] factors like the substantial financial commitment that the government has to make for Hong Kong's future development, and also the geopolitical situation that has brought drastic changes to economies worldwide, the impact on Hong Kong's economy and the continual lack of a definite, clear outcome of those geopolitical tensions,” she said. Yeung pointed out that the SAR government ranked second in efficiency according to last year’s World Competitiveness Yearbook published by the Swiss-based IMD World Competitiveness Center, saying this reflects the excellent quality of Hong Kong’s civil servants. A reasonable salary adjustment will help keep up civil service morale and also serve as recognition for their work, she added. Yeung said she would meet with representatives from several civil servant unions on Wednesday to gather feedback before the Executive Council makes a final decision. The final proposal will then be submitted to the Legislative Council's Finance Committee for vetting. A pay trend survey recently pointed to a 4.12 percent and 2.64 percent pay rise for senior and middle-ranking civil servants, respectively.
